Hart of Autism Camp Application
Hart of Autism is extremely excited about Summer Day Camp. Our group is growing and so are our children.

This Camp is for the Special Need Child, not their siblings.
We will have six (6) days of camp; however, there will be two (2) groups "Adventures Within" and "Out & About". Each group will attend camp for three (3) days.
"Adventures Within" will attend camp on Wednesday,  June 28th, Thursday, June 29th, and  Friday, June 30th  from 9:00 a.m. to 2:00 p.m

"Out & About" will attend camp on Wednesday,  July 19th, Thursday, July 20th, and Friday, July 21st  from 9:00 a.m. to 2:00 p.m.
Camp is at the Munfordville Baptist Church, located at 501 North West Street, Munfordville, KY 42765
Once registration is complete, we'll reach out to you to let you know what camp group your child will be included in.

Please fill out the attached Camp Application completely with details of your child's concerns and needs for our therapist to review and choose the best match for your child's camp placement. Our therapist will review each application to assure your child is placed in the best camp for their needs and safety.  Safety is our foremost concern.
We will have limited spots available for both camps. The application must be submitted online and a $25.00 non-refundable check for the registration fee must be submitted immediately by Paypal before the application will be reviewed. Once applications and registration fees have been received and the application has been reviewed for placement and we will email you a welcome letter. If all spots have been filled for both camps, we will place your camper on our waiting list for any open spots due to cancelation.   "Out & About" Campers will need to bring lunch money each day!!
"Adventures Within" Campers will need to bring their lunch each day!!
